By subscribing, I agree to these Subscription Terms and Policies.
Plan Options and Pricing:
Basic Whitening Membership: $25 per month.
Premium Whitening Membership: $40 per month.
Botox Add-On (available only with Premium): $15 per month (total $55/month for Premium + Botox).
All plans require a 12-month minimum commitment. After 12 months, the subscription continues month-to-month until canceled.
Your monthly price is locked in for the initial 12-month commitment period. After this period, prices are subject to change with 30 days’ notice via email.
Benefits:
Benefits are as described in the membership plan (e.g., annual whitening sessions, take-home kits, Botox treatment, discounts).
Unused benefits do not roll over and expire at the end of each subscription year (anniversary date).
Benefits are non-transferable and must be redeemed at Galax Family Dental by the subscriber.
Botox treatments are administered by a licensed Dentist and require a consultation; results vary and are not guaranteed.
Billing:
Charged monthly via Stripe to the provided payment method.
Taxes (if applicable) are added at checkout.
Failed payments may incur a $5 retry fee
Refunds: No refunds for partial months or unused benefits.
Cancellation Terms:
During the 12-month commitment: Cancellation is not permitted except for extenuating circumstances (e.g., medical reasons, verified in writing). Early termination incurs a fee equal to 50% of the remaining months’ charges.
After 12 months: Cancel anytime by contacting us in writing (email/phone). Cancellation takes effect at the end of the current billing cycle (no prorated refunds).
To cancel: Notify us at least 5 business days before the next billing date.
Upon cancellation, access to benefits ends immediately, and any scheduled appointments may be rescheduled at standard rates.
Other Policies:
Changes: After 12 months, you can upgrade/downgrade plans by contacting us. Changes take effect the next billing cycle.
Pausing: Subscriptions cannot be paused.
Privacy: We handle your data per our privacy policy and Stripe’s terms. Payment info is securely stored by Stripe.
Liability: Galax Family Dental or Byron Palmer, DDS are not liable for allergic reactions, dissatisfaction with results, or Stripe-related issues. Botox and whitening involve risks—discuss with your provider.
Governing Law: This agreement is governed by the laws of Virginia
